As an insurer, a new circular from the "Commissariat Aux Assurances" requires us to have the most recent identity data concerning our customers. In order to meet these legal obligations, could you send us a copy of both sides of your identity card and complete the following information:

Nationality of policyholder*

Customer number (see "Your reference" at the top left of the letter)*

Profession of the policyholder *

If the policyholder is not the insured - complete all of the following information
