- Information on the parties involved: indicate in columns A and B the respective information regarding the driver, the vehicle and the insurance for each party. It doesn’t matter which column your information goes in, but make sure that the information provided by the other party is correct.
- Depiction and description of the damage: make a drawing showing how the accident happened and indicate the damage to the vehicles with arrows. In addition, take photos of the damage and send them to your insurer.
- Accident circumstances: In the middle column, tick the boxes that apply to your situation. If there are none that correspond to your situation, add explanations in the “observations” section.
